• Complete our secure online referral form with the patient's information and reason for referral.

• 2. Our Team Reviews the Referral

• Referrals are reviewed within 1 business day.

• 3. We Contact the Patient

• Our team will typically contact the patient within 1–2 business days to assist with scheduling and answer any questions about getting started.

• 4. We Coordinate Care

• When appropriate and with the necessary patient authorization, we communicate with referring providers to support continuity of care and collaborative treatment.
