Office Policies

Confidentiality Policy:
At Bodhi Wellness, we prioritize the privacy of our patients. To uphold this commitment, HIPAA privacy notices will be provided and must be signed by the patient before any care is initiated.
Controlled Substance Policy:
At Bodhi Wellness, we offer controlled substances when deemed necessary for managing your mental health diagnosis. All patients must sign a controlled substance contract, which may include random drug screenings throughout treatment. Before receiving any controlled substances, patients are required to complete a drug screening. The provider reserves the right to terminate the patient-provider relationship if any illicit substances are detected in drug screenings.
Financial Agreement Insurance Information:
Patients must ensure they have active insurance coverage mental health services, including outpatient care and medication management, that Bodhi Wellness is in-network with their plan. Co-payments, coinsurance, and deductibles will on the benefits outlined by the patient's insurance provider. If insurance denies benefits, patients are responsible for any outstanding balances. Additionally, the patient or responsible party must pay any balance owed, regardless of the reason for the insurance denial, and patients are encouraged to request reimbursement from their insurance company as needed.
No-Show/Cancellation Policy:
Please remember to cancel your appointment at least 24 hours in advance. Fees will occur for no-shows and cancellations now made in a timely manner.
Payment is due upon request of the service.
Patient Dismissal Policy:
Patients have the right to end their treatment with the provider at any time and for reason. Similarly, the may terminate the patient relationship when necessary. Reasons for termination can include, but are not to, not following treatment plans, disruptive or abusive behavior, failure to make payments, substance use or misuse, lack of progress need for treatment, repeatedly missing appointments or cancel late, or violations of practice policies and provider limitations.
Payment Policy:
All patients are required to have a valid credit card on file for appointment fees.
Prescription Policy:
Bodhi Wellness, prescription refill requests can only be made during appointments. Please note that medications will not be sent to the pharmacy until all outstanding balances are settled and follow-up appointments with the provider are scheduled. If a prescription needs prior authorization, patients should be aware that this process may take up to three business days and will be managed in the order they are received.

We do not share, sell, rent, or trade any information provided with third parties for promotional purposes.As a current or prospective customer, you understand that you can text us STOP at any time to opt out of receiving SMS text messages from us. You can text us HELP at any time to receive help.You understand that the messaging frequency may vary. Messaging & data rates may apply.Your mobile information will not be shared with any third parties/affiliates for marketing/promotional purposes.
